APPLICATION:

-application Shall be made using Form Official (1/97 MDS.PKDM amended 2015) which can be obtained at the counter, Level One, the Selama District Council District Council During or through the website: www.mdselama.gov.my.

Official -Form amendments MDS.PKDM 1/97 2015 can be obtained at the counter, Level One, the Selama District Council.

-application Shall be accompanied by copies of academic certificates, identity cards, birth certificate / birth certificate, which has been confirmed and a recent passport size photograph.

-Application Candidates who are serving in Government / Local Authority / Statutory Bodies shall be made by the Head of their department and make sure a copy of the Statement of Service was updated and approved by the department head attached. Applications not according to the rules in the form will be rejected.

-Application Completed documents must be submitted: -
