Applicants must use the Application Form (M.P.N.) 1/01 can be found in the Management Services Department, Nilai Municipal Council worth RM1.05 (including GST 6%). The completed form must be submitted together with: -

i) A copy of the identification card;
ii) Copy of Birth Certificate
iii) passport sized photograph (affixed on the front page of the form);
iv) Copies of certificates required for eligibility decisions

All copies of the certificate does not need to be confirmed by Government officials Group
Management and Professional / Village Chief / Headman / Commissioner for Oaths / Members
