a. Passport size color photograph;
b. My card / ID card;
c. Birth certificate (birth certificate);
d. School leaving certificate;
e. Sijil Pelajaran Malaysia (SPM);
f. Relevant certificates.
The name of the office must be written at the top left of the envelope and form
