i. Identification card;
ii. SPM / MCE, HSE / STPM or equivalent;

iv. Confirmation letter from the employer of the existing / prior to taking into consideration work experience / wage; and
v. A copy of the updated statement of service (for employees of Government / Statutory Body / Semi-Government is being served).
