All applications must use the application form MDB.1 worth RM3.00 Designation can be obtained at the Department of Administration Bachok District Council or send a self addressed envelope measuring 32 cm x 22 cm and money valued at RM3.00 on behalf of the President Bachok District Council City Islamic Tourism and its stamp to the address below.
