The completed form must be submitted together with. All copies of the certificate does not need to be confirmed by government officials and Professional Management Group / Village Chief / Headman / Commissioner for Oaths / MP / Member of the Legislative Assembly. :

i) A copy of the identification card;
ii) Copy of Birth Certificate
iii) passport sized photograph (affixed on the front page of the form);
iv) Copies of certificates required for eligibility decisions
