Applicants must use Form MDSB cost RM2.00 per form available from the Department of Management Services, or can be downloaded via the Web site Sabak Bernam District Council on the links below. Application forms must be accompanied by a copy of: -

i) identification card and birth certificate.
ii) Copies of schooling certificates including Secondary School Leaving Certificate,
iii) Copies of academic qualifications (Please attach a letter of recognition of qualifications from the Public Service Department (PSD) for Certificate / Diploma / Degree from Institution and abroad - (If applicable)
iv) the results of all semester transcript
v) A passport photo (color).
vi) Each application must be inserted into the envelope and the name of the post applied for should be noted on the left side of the envelope.
